I think both of those are due to his low m2m skills - his feet are getting tangled up as he's trying to do man coverage.

Looking through the game log (not analyzing it, but quickly glancing at it) I see that San Antonio seems to be 100% blitz - which, when you do that in 0.4.1 - especially if those blitzes have been scouted or you are using the same blitz over and over - you're going to get these kinds of results.
