The injury rate is at the max - 100%. Useful for testing.
Questionable and above are basically depth.
JDB has already said you should expected players to be dinged up and play through injuries over the course of the season.
28 injuries in the game seems a tad high but overwhelming majority were "able to return to game". A number on other injuries mostly late on game, guess played with injuries, made them worse possibly.
But no, not rigged.

I think there's scope for a hardcore league, injuries 200%, 1 pre season game etc... a real challenge.

Just play a little bit with Fatigue settings. Especially the big boys live longer when they rotate earlier.

Same, lost my starting RG in last game to join 3 other starters that are out for the POs. And my fatigue settings are low for both OL and DL. The offense has been hit hard, best RB,WR, and TE are gone. Worst batch of injuries before PO that I have experienced in all my time playing this game. Unreal, like what happened to Denver before the SB, losing Nix. May have to pick up FAs to prevent the auto manager from doing something ridiculous.

I want confirmation fatigue affects injuries before I tinker with my sliders again. People reported that weight affects injuries as well but I have never seen any correlation between the two in regards to injuries

No, there is no hidden injury prone attribute. During the play there are a multitude of "injury creating" events, the probability that any one event creates an injury is a factor of fatigue, conditioning, and current injury level

A player's fatigue level is dynamic throughout the play, and increases based on his conditioning and weight. His top speed constantly is re-calculated based on his fatigue - so a player with lower conditioning and more weight will slow down earlier than a small player in top conditioning.
