FF, I don't think that this is an issue of access. If someone wanted to hack your team they'd bench your top QB and RB.

Could be if they wanted to mask it as an AI thing. But I agree, it's more likely the game decided it needed to correct some stuff

I'm not sure when I'm gonna get around to getting everything back to where it should be, it's a lot of work and I'm pretty busy right now :(

Another possible option is that some inside run plays are actually marked as outside run plays. If you give me a list of play id's I can tell you what play was ran and what type of play it was (info that hopefully will make its way to the game log)

Ok...a bit tough to do this but here are some examples.

By way of comparison, and this is just one example:
This play looks like it could be an off tackle but ends up being a sweep.

And this play looks like a sweep and ends up running inside tackle.

I'm wondering if the play diagrams don't actually coincide with the correct plays now since I'm seeing this go both ways. Plays that are designed as off tackle handoffs are ending up being sweeps while sweep plays are going inside.

I understand the runner might have some discretion you can tell immediately upon snap that the runner isn't doing what he's supposed to. Could this be a discipline issue? Does that affect a player's diligence in running his assignment?

This is one reason why it would be really important for us to see the name of the play when watching the game... to know what's going on. It's nice to watch the game, but it's even nicer to understand what is going on in the game. It's really hard to make adjustments when you aren't always sure what play was just run. Just put the name of the play in the corner of the replay... and make different names for different plays, which includes the formation. example... Single Back HB Dive Strong, I Formation HB Dive Strong, etc. etc. Same with defense, and each team can only see their play call. So, when I'm watching my games, I only see my play call on offense and defense.

This way, we know what's going on with our teams, and we are able to make adjustments.

I've asked for this in the past, even did a nice mockup in Photoshop :)

However I don't see the reason to hide the opposing team's play. Think of it like NFL teams watching film on their opponent. They know the exact play call, formation, routes, how it was read etc etc.

That's the biggest challenge of the game.

What would really be great would be to be able to modify our own plays. So take the example of me being able to spot the tendency of my opponent to blitz +2 outside on 3rd and 5 or more I could modify route of my TE to run a 6 yard button hook.

I agree 100% - and hopefully I'll get the plays more visible than they are soon. I love nero's idea, I'm excited to get into the next gen where I can build some of these more advanced ideas!
